Fatty Liver Diet Plan: Foods to Eat, Avoid and a 7-Day Pakistani Chart
Fatty liver is one of the most common findings on ultrasound reports in Pakistan today. Many people are told they have “grade 1” or “grade 2” fatty liver during a routine check-up, often with no symptoms at all, and are left wondering what to eat. The good news is that fatty liver can usually be improved, and often reversed, with the right diet and some weight loss. This fatty liver diet plan explains what fatty liver is, which Pakistani foods help or harm your liver, and gives you a simple 7-day meal plan.

Key takeaways
- About 3 in 10 Pakistani adults have fatty liver, and more than half of people with diabetes.
- Fatty liver can usually be improved or reversed: losing 7 to 10% of your body weight clears liver inflammation in most people.
- Cutting sugar, sweet chai and cold drinks is the single most important diet change.
- Choose atta roti over naan and paratha, fill half your plate with sabzi, and cook with 1 to 2 tablespoons of oil for the whole dish.
- Avoid alcohol and unproven liver tonics, and ask your doctor about testing for hepatitis B and C.
What Is Fatty Liver?
Fatty liver means too much fat has built up inside your liver cells. The most common type is not caused by alcohol. It used to be called non-alcoholic fatty liver disease (NAFLD), and doctors now call it metabolic dysfunction-associated steatotic liver disease (MASLD), because it is closely linked to weight, blood sugar and cholesterol.
In many people, fatty liver stays mild. In some, the fat causes inflammation (steatohepatitis), which over years can lead to scarring (fibrosis), cirrhosis and even liver cancer. That is why it is worth taking seriously, even when you feel well.
What do grade 1, 2 and 3 mean?
Ultrasound reports in Pakistan often grade fatty liver by how much fat the scan shows:
- Grade 1 (mild): a small amount of fat. Usually very reversible with diet and exercise.
- Grade 2 (moderate): more fat. Still reversible, but needs steady lifestyle changes.
- Grade 3 (severe): a lot of fat. Needs close follow-up with your doctor.
The grade shows the amount of fat, not how much inflammation or scarring there is. Your doctor may suggest blood tests or a FibroScan to check your liver further.
How Common Is Fatty Liver in Pakistan?
Very common. A 2024 review of 34 Pakistani studies found that about 3 in 10 adults in the general population have fatty liver. The rate was about 58% in people with diabetes, 47% in people with obesity and 74% in people with high blood pressure. (Source: Scientific Reports, 2024)
What Causes Fatty Liver?
- Being overweight, especially with fat around the stomach.
- Type 2 diabetes, prediabetes or insulin resistance.
- High triglycerides or cholesterol.
- A diet high in sugar, sweet drinks, white flour, fried food and oil.
- Sitting most of the day with little exercise.
- PCOS and an underactive thyroid in some people.
- Rapid weight loss, some medicines, and alcohol.
South Asians can develop fatty liver even at a normal body weight, because we tend to store fat around the organs. If your waist is large, you can be at risk even if your BMI looks normal.
Symptoms and Tests
Most people with fatty liver have no symptoms. Some notice tiredness or a dull ache or heaviness on the upper right side of the stomach. It is usually found by chance on an ultrasound or when liver tests (ALT, AST) are slightly raised.
Because hepatitis B and C are also common in Pakistan and can damage the liver, ask your doctor whether you should be tested for them too. Do not assume every liver problem is “just fatty liver”.
Can Fatty Liver Be Reversed?
Yes, in most cases, and weight loss is the most powerful treatment. In a study of 293 people with fatty liver inflammation, those who lost 7 to 10% of their body weight saw the inflammation clear in about two-thirds of cases, and those who lost 10% or more saw it clear in about 9 out of 10. For an 80 kg person, 7 to 10% means losing about 6 to 8 kg, slowly, over several months. Even 3 to 5% can reduce the fat in your liver. Aim for 0.5 to 1 kg a week. Losing weight too fast, through crash diets or very long fasts, can make fatty liver worse.

Fatty Liver Diet: 8 Rules That Work
1. Cut sugar and sweet drinks first
Sugar, especially from cold drinks, packaged juices, sweet chai and desserts, is turned into fat by the liver. This is the single most important change. Move to chai with less sugar or none, and replace sweet drinks with water, lemon water or lassi without sugar.
2. Choose whole grains over white flour
Have atta roti instead of naan, paratha, white bread and bakery items. Keep rice to half a cup to one cup per meal, and add daliya, oats, chana and daal for fibre.
3. Fill half your plate with sabzi
Vegetables such as palak, lauki, tori, karela, bhindi, gobi, cabbage and salad are low in calories and high in fibre, which helps both weight and blood sugar.
4. Use less oil and better fats
Fried and oily food adds a lot of calories. Cook with 1 to 2 tablespoons of oil for the whole family dish, choose canola, olive, sunflower or mustard oil over ghee and banaspati, and skip the extra tarka. Grill, bake or air fry instead of deep frying.
5. Eat protein at every meal
Eggs, chicken, fish, daal, chana, lobia and dahi keep you full and protect your muscles as you lose fat. Fish such as rohu, surmai or salmon twice a week also gives omega-3 fats, which are good for the liver.
6. Cut processed and fried snacks
Samosa, pakora, nimko, chips, biscuits, cake and fast food are high in fat, salt and refined flour. Keep them for rare treats.
7. Avoid alcohol completely
Any alcohol adds extra strain to a fatty liver. If you have fatty liver, the safest amount is none.
8. Be careful with “liver detox” products
Herbal liver tonics, detox teas and supplements are not proven to treat fatty liver, and some herbal products can actually damage the liver. Do not take them without asking your doctor.
What About Coffee and Green Tea?
Several studies have found that people who drink coffee regularly tend to have less liver scarring. If you already drink coffee, 2 to 3 cups a day without sugar or cream is fine for most people. Green tea without sugar is also a good choice instead of sweet drinks. Neither is a treatment on its own, and you do not need to start drinking them.

Exercise for a Healthier Liver
Exercise reduces liver fat even before you see much weight loss on the scale. Aim for at least 150 minutes a week of brisk walking, cycling or swimming, which is about 30 minutes on 5 days, plus strength exercises 2 to 3 times a week. If most of your weight is around the stomach, our guide on how to lose belly fat has a simple home routine.
Fatty Liver with Diabetes
More than half of people with type 2 diabetes in Pakistan have fatty liver, and each condition makes the other worse. Controlling your blood sugar, losing weight and following a low-sugar diet helps both. See our diabetes diet chart, and ask your doctor about medicines that may also help your liver.
High uric acid often comes with fatty liver. See our uric acid diet for foods to eat and avoid.
Frequently Asked Questions
Can grade 1 fatty liver be cured?
In most people, yes. Grade 1 fatty liver is mild and often goes away with weight loss, less sugar and fried food, and regular exercise. Many people see an improvement on their ultrasound within 3 to 6 months of steady changes.
How long does it take to reverse fatty liver?
It depends on how much fat there is and how much weight you need to lose. With a weight loss of 0.5 to 1 kg a week, liver fat often improves within a few months. Ask your doctor when to repeat your ultrasound or liver tests.
Is roti or rice better for fatty liver?
Both can be eaten in controlled portions. Atta roti has more fibre than white rice, so it is usually the better everyday choice. Keep it to 1 roti or half a cup to one cup of rice per meal, with plenty of sabzi and protein.
Which fruits are good for fatty liver?
Guava, apple, orange, pear, papaya, jamun and berries are good choices. Have 2 servings a day as whole fruit, not juice. Mango, grapes and dates are higher in sugar, so keep portions small.
Are eggs safe if I have fatty liver?
Yes. Eggs are a good source of protein and can be part of a fatty liver diet. One to two eggs a day, boiled or cooked with little oil, are fine for most people. If you also have high cholesterol, ask your doctor or dietitian how many suit you.
Is milk bad for fatty liver?
No. Low-fat or toned milk and dahi are fine. Choose them over full-fat versions, and avoid sweetened milk drinks and milkshakes with added sugar.
Do liver tonics or detox drinks help fatty liver?
No. There is no good evidence that liver tonics, detox teas or herbal supplements treat fatty liver, and some herbal products can harm the liver. Weight loss, less sugar, regular exercise and your doctor’s advice are what work.
